Terms of use

Effective from: 15 September 2026. In case of discrepancies, the Polish version prevails.

§ 1. General

  1. These terms set out the rules for using the website at fizjo-fan.pl (the "Website").
  2. The Website is owned and operated by Fizjo-Fan Robert Bulik, Aleje Jerozolimskie 200, 02-486 Warsaw, Poland, NIP (tax ID): 8371722832 (the "Provider").
  3. Contact: fizjofan88@gmail.com, tel. +48 780 646 601.
  4. Use of the Website is free of charge and constitutes acceptance of these terms.

§ 2. Nature of the information

  1. The Website is informational – it presents the services of the Fizjo-Fan physiotherapy clinic, prices, contact details and educational articles.
  2. Content published on the Website, including educational articles, is general information and does not constitute medical advice, diagnosis or a treatment recommendation. It does not replace a consultation with a physiotherapist or doctor.
  3. The price list is for information only and does not constitute an offer within the meaning of Art. 66 of the Polish Civil Code. The binding price is confirmed when the visit is booked.

§ 3. Booking

  1. Visits are booked via the external Booksy system (operated by Booksy International sp. z o.o.), by phone or by e-mail. Use of Booksy is subject to Booksy's own terms.
  2. A booking is confirmed by Booksy or by the Provider. The contract for the physiotherapy service is concluded when the appointment is confirmed.
  3. Visits may be cancelled or rescheduled free of charge at least 24 hours in advance – via Booksy, by phone or SMS. [TO BE CONFIRMED BY THE CLIENT: any fee for late cancellation / no-show.]
  4. Payment is made on site, in cash or by card, after the service. Invoices are issued on request.
  5. Services are provided to adults. Minors may use the services with the consent and in the presence of a legal guardian.
  6. The Provider may refuse to provide a service in the event of medical contraindications, signs of alcohol or drug use, or behaviour that breaches accepted social norms.

§ 4. Technical requirements

A device with internet access and an up-to-date browser with JavaScript and cookies enabled (to the extent described in the Privacy policy).

§ 5. Copyright

  1. All content on the Website – texts, photos, graphics, logos, layout – is the property of the Provider or used under licence and is protected by copyright law.
  2. Copying, distributing or otherwise using the Website's content without the Provider's consent is prohibited, except for permitted personal use.

§ 6. Liability

  1. The Provider makes every effort to keep the information current and accurate but does not guarantee its completeness or uninterrupted availability of the Website.
  2. The Provider is not responsible for the content or operation of external websites linked from the Website (Booksy, Google Maps, Facebook, Instagram).
  3. Users must not supply unlawful content or take actions that could disrupt the Website.

§ 7. Complaints

  1. Complaints about the Website or the services may be submitted by e-mail to fizjofan88@gmail.com or in writing to the Provider's address.
  2. A complaint should include contact details and a description of the issue. The Provider responds within 14 days of receipt.
  3. Consumers may use out-of-court dispute resolution, including the EU ODR platform (ec.europa.eu/consumers/odr).

§ 8. Personal data

The rules for processing personal data and using cookies are described in the Privacy policy.

§ 9. Final provisions

  1. The Provider may amend these terms. Amendments take effect upon publication on the Website and do not apply to visits booked before the amendment.
  2. Matters not covered here are governed by Polish law, in particular the Civil Code, the Consumer Rights Act and the Act on the Provision of Electronic Services.
